Important reminder regarding NYS required health requirements

Physicals for grades K, 1, 3, 5, 7, 9, 11 - valid for one year from September 3, 2025. Required within 30 days of school starting

Required vaccines for the following grades-

All Vaccines are required by September 16, 2026 to remain in school.

Click here for the 2025-26 School Year New York State Immunization Requirements for School Entrance/Attendance.

Kindergarten- DTap- 5 doses

Hepatitis B- 3 Doses

Polio- 4 doses

MMR- 2 doses

Varicella- 2 doses

NOTE: MMRV is acceptable as well with 2 doses

6th Grade- TDap after 10th birthday
