SWR Athletics Registration System

Shoreham-Wading River Central School District utilizes FinalForms, a secure online platform that simplifies the athletic registration process for our Wildcat families while improving administrative efficiency. FinalForms tracks and securely stores your family's information, making registration seamless year after year.

BEFORE YOU REGISTER:

Please have electronic copies of updated physical forms, medical orders, and immunization records ready to upload during the clearance process to ensure your student-athlete’s file is complete.

INFORMATION NEEDED TO REGISTER:

Past medical history and physical condition details

• Current medications and health management plans
• Allergy information
• Emergency contact information and preferred medical facilities

REQUIRED ONLINE SIGNATURES:

To complete the registration, SWR requires electronic signatures from both:

• Parent/Guardian: Must be 18 years of age or older.

• Student-Athlete: To acknowledge the Student Code of Conduct and Concussion Fact Sheet.

Please ensure both the parent/guardian and student are present to complete and sign the registration.

REGISTRATION PROCESS:

1. Access the Portal: Click here to go to the Shoreham-Wading River FinalForms site
2. Create or Log In to Your Account:
   • New Users: Click New Account under the Parent icon. Enter your name, birthdate, and email address, then click Register. You will receive a confirmation email to activate your account.
   • Returning Users: Click Log In under the Parent icon and sign in with your credentials.
3. Add or Select Student: Click Add Student (or select your existing student-athlete) and enter the required demographic and school information.
4. Select Sport & Complete Forms: Choose the sport(s) your child plans to play for the upcoming season. Complete all required fields (marked in red).
5. Sign & Submit: Review the registration summary, provide the required electronic signatures, and click Submit. You will receive a confirmation email once your forms are successfully submitted.

Note: You can log back into FinalForms at any time to update contact details, upload new medical forms, or check your student's clearance status.
