Physical Exam & Immunization Requirements - Reminder
Dear Parents and Guardians,
The Shoreham-Wading River School District would like to acquaint you with policies related to student physical exams and immunization requirements. It is the parent’s/guardian’s responsibility to have their child(ren) up to date with their vaccinations and physicals. If you are not sure, please contact your physician or the school nurse.
Physical Examinations:
New York State requires that all kindergartners, students entering grades 1, 3, 5, 7, 9, 11, and all new entrants into the district have a health examination. Completed physical examination forms must be submitted to the health office within 30 days of the start of school. Sports physicals satisfy the 7th, 9th, and 11th grade physical requirements. A health examination done within 12 months of the first day of school will also satisfy this policy. If written medical documentation of health examination is not received within 30 days of the start of school, the student will be scheduled to be examined by the school physician.
Immunization Requirements:
Please click here for school entrance/attendance immunization requirements. To avoid non-compliance and possible removal from school, your child’s immunizations and documentation must be current.
Miller Avenue School:
Kindergarten – Students entering kindergarten must have their second MMR and second Varicella. Please check with your pediatrician to ensure that your child is up-to date on their vaccinations for entering kindergarten.
Albert Prodell Middle School:
Grade 6 - Students entering 6th grade who have reached 11 years of age must receive a Tdap vaccine. Ten-year-old students who are entering 6th grade are expected to receive a Tdap vaccine when they turn 11 years old.
Entering Grade 7 – Students entering 7th grade must receive one dose of Meningococcal vaccine before the first day of school. If your child had their first dose as a 6th grader, then another dose is not required until entering 12th grade.
SWR High School:
Entering 12th grade – Students entering 12th grade must receive their second dose of Meningococcal vaccine before the first day of school.
BETWEEN NOW AND THE FIRST DAY OF SCHOOL, PLEASE MAIL, EMAIL, OR DROP OFF YOUR CHILD’S PHYSICAL AND IMMUNIZATION DOCUMENTATION AS SOON AS THEY ARE COMPLETE.
